CareSearch Loyalty Rewards Program

                                                                                                                                                      I.SUMMARY

Subject to the terms and conditions below, CareSearch Recommended Caregivers (members who are in good standing with CareSearch) can earn Rewards Points for the following:

  1. Becoming a CareSearch Recommended Caregiver
    1. Every Caregiver who is currently in good standing and a Member of CareSearch receives 250 Rewards Points
    2. Any New Caregiver who meets/passes our standards/interview and becomes a Member of CareSearch receives 250 Rewards Points
  2. Hired by a Family who utilizes CareSearch Services
    1. Caregivers who respond to Job Posts and are subsequently Hired by the Family will receive Rewards Points after completing their first shift
    2. Rewards Points are determined by the needs of the Family
  3. Continued Education
    1. Online Continued Education Courses approved / validated by CareSearch are eligible for points
    2. Facilities which offer Continued Education approved / validated by CareSearch are eligible for points
  4. Family Referral
    1. Members are awarded Rewards Points after the 1st month of service the Family utilizes CareSearch Services
    2. Rewards Points are determined based on the number of hours the Family books on average per week
  5. Fellow Caregiver Referral
    1. Members are awarded Rewards Points for every Caregiver they refer to CareSearch who meets and passes CareSearch standards/interviews and who becomes a member of CareSearch
    2. Members are also awarded Rewards Points for the 1st Qualified Family their referred Caregiver works for
  6. Referred Caregiver is hired by Family A which qualifies them for 300 Rewards Points
    1. Member who referred said Caregiver also receives 300 Rewards Points
    2. Only valid for the FIRST Family the Referred Caregiver is hired through

II.Rules and Regulations

The purpose of this guideline is to establish a CARESEARCH LOYALTY REWARDS PROGRAM (the “Program”) designed to recognize and express appreciation to CareSearch Recommended Caregivers for their service.

A.                Eligibility

The Program is available to all CareSearch Recommended Caregivers, defined as members who are in good standing with CareSearch as solely defined by Senior Match Solutions Corp..

B.                 Ability to Earn Rewards

If a participating CareSearch Recommended Caregiver is no longer in good standing with CareSearch, and/or there is any fraud or abuse related to the accrual of points, Senior Match Solutions Corp. reserves the right to prohibit such a CareSearch Recommended Caregiver from earning points through that CareSearch Recommended Caregiver’s account until the CareSearch Recommended Caregiver returns to a member in good standing.

Failure to maintain good standing is grounds to cause such a CareSearch Recommended Caregiver to forfeit any points in that CareSearch Recommended Caregiver’s account, subject solely to the discretion of Senior Match Solutions Corp. If points are forfeited for any reason, Senior Match Solutions Corp. will not reinstate these points to a CareSearch Recommended Caregiver’s

III.Earning Rewards

The following is subject to change at any time without notice to CareSearch Recommended Caregivers by Senior Match Solutions Corp. at its sole discretion.

A.                Earning Rewards

Rewards are earned as points, awarded as follows:

  1. Becoming a CareSearch Recommended Caregiver
    1. Every Caregiver who is currently in good standing and a Member of CareSearch receives 250 Rewards Points
    2. Any New Caregiver who meets/passes our standards/interview and becomes a Member of CareSearch receives 250 Rewards Points
  2. Hired by a Family who utilizes CareSearch Services
    1. Caregivers who respond to Job Posts and are subsequently Hired by the Family will receive Rewards Points after completing their first shift
    2. Rewards Points are determined by the needs of the Family
  3. Continued Education
    1. Online Continued Education Courses approved / validated by CareSearch

  6. Referred Caregiver is hired by Family A which qualifies them for 300 Rewards Points
    1. Member who referred said Caregiver also receives 300 Rewards Points
    2. Only valid for the FIRST Family the Referred Caregiver is hired through

B.                 Conditions

  1. Family Needs which are more difficult to fill (based on hours, pay, duties, etc) may be awarded more Rewards Points
  2. Rewards Points will be determined and advertised in Job Post prior to interview with Family.
  3. Points earned are not transferable, have no cash value, and cannot be used as payment of obligation to anyone.
  4. If there is any abuse of the Program, failure to follow Program terms, or any misrepresentation by a CareSearch Recommended Caregiver, all unredeemed points shall be forfeited and no additional points shall accumulate.
  5. Although there is no maximum number of points that a CareSearch Recommended Caregiver can accumulate in the Program, Bonus/Promotional offers, if any, may have a maximum accumulation.

C.                Expiration:

Points earned in this Program will not expire.

IV.Redemption Rules

A.                Redemption Process:

  1. Points are not available for redemption until they are posted to a CareSearch Recommended Caregiver’s account.

  3. Senior Match Solutions Corp. has no other obligation with respect to the Program beyond those described in these and the Redemption Rules.

B.                 Ability to Redeem Rewards:

  1. If a CareSearch Recommended Caregiver’s Account is in default as described herein and/or a CareSearch Recommended Caregiver’s Account is closed for any reason and/or there is any fraud or abuse related to the accrual of a CareSearch Recommended Caregiver’s rewards, Senior Match Solutions reserves the right to prohibit such a CareSearch Recommended Caregiver’s from redeeming rewards through that CareSearch Recommended Caregiver’s Account until that CareSearch Recommended Caregiver’s Account is no longer in default, and to cause such a CareSearch Recommended Caregiver to forfeit any rewards in that CareSearch Recommended Caregiver’s Account.
  2. If a CareSearch Recommended Caregiver’s rewards are forfeited for any reason, Senior Match Solutions Corp. will not reinstate these rewards to that CareSearch Recommended Caregiver’s Account.
  3. Minimum redemption amounts may apply and will be disclosed to the a CareSearch Recommended Caregiver prior to the CareSearch Recommended Caregiver’s redeeming of such points.
  4. Individual merchants and service providers are wholly responsible for the quality and performance of any goods and/or services received as a benefit of this Program, if any, including those received through redemptions and promotions.
  5. Such merchants and service providers, if any, are not affiliated with Senior Match Solutions Corp., nor are the listed merchants considered sponsors or co-sponsors of this Program.
  6. Use of merchant names and/or logos is by permission of each respective merchant, and all trademarks are the property of their respective owners.
  7. Items are subject to availability and may be substituted with items of equal or greater value as necessary.
  8. Redemptions are final.
  9. Please check the applicable merchant’s terms and conditions before you redeem.
  10. Any rules of the merchants relating to returns and exchanges of rewards apply.
  11. Merchants participating in the Program are subject to change.
  12. The CareSearch Recommended Caregiver is responsible for any tax liability, including disclosure requirements, related to participating in the Program (for example, the receipt of redemption items).

V.FINAL ITEMS

A.                NO PURCHASE NECESSARY

Unless otherwise noted, no purchase is necessary to accrue points hereunder.

B.                 APPLICABLE LAW (VOID WHERE PROHIBITED)

The Program is subject to and governed by applicable federal, state and local laws and regulations. Participation in the Program is void where prohibited or otherwise restricted by law.

C.                PROGRAM CANCELLATION, SUSPENSION OR MODIFICATION

Senior Match Solutions Corp. reserves the right to cancel, terminate, modify or suspend the Program, at its sole discretion. The decision of Senior Match Solutions Corp. to cancel, terminate, modify or suspend the Program shall be final in all respects

D.                NO OBLIGATIONS

Senior Match Solutions Corp. has no other obligation with respect to the Program beyond those described herein. These Rules supersede any and all prior versions of the Program Redemption Rules, whether written or oral.

E.                 GOVERNING LAW

The Program shall be governed by and construed in accordance with the laws of the State of Texas, without reference to its conflict of laws principles. By participating in the Program, CareSearch Recommended Caregivers hereby submit to the jurisdiction and venue of the federal and state courts of Texas in Austin, Travis County, Texas having subject matter jurisdiction and waive the right to have any dispute arising out of the subject matter hereof adjudicated in any other forum. In no event, will any CareSearch Recommended Caregiver be entitled to injunctive relief or equitable relief of any kind, or restrain the continuation of the Program.
